Transitional Housing Placement Program (THPP)

Overview
Our Transitional Housing Programs help current and former foster youth learn and develop the skills they need to live independently.  Placements of teenage foster youth can be unstable, causing them to require extra support and to help them learn how to be an independent adult.  We offer a safe place with caring and loving adults who help them successfully transition to adulthood.

Youth can choose to live in an apartment by themselves, with a roommate, or even choose to live with a host family who can support and guide them in their transition into adulthood.  Our team of caring professionals helps teach them how to budget money, handle their responsibilities, take care of an apartment or home, balance work, education and play, and how to give back to the community.  Depending upon their age, these young adults may participate in one or more of the programs below until they reach age 24.

What is Transitional Housing Placement Program (THPP)?
The Transitional Housing Placement Program (THPP) is for youth who are 16 to 18 years of age and are in foster care. The purpose of THPP is to provide a safe living environment to youth while helping them develop the skills necessary to live independently after leaving foster care. A goal-oriented Transitional Independent Living Plan (TILP) is developed by the youth, their support persons, and a social worker or probation officer. The TILP outlines goals and objectives that the youth will work toward to achieve higher levels of self-sufficiency.

Who is THPP for?
THPP is for youth that are currently in foster care, between the ages of 16 to 18, are working toward TILP goals, and are involved in Independent Living Program Services.

Where do the youth live?
Youth in THPP are either living with a Host Family or at a Single Site. A Host Family is where the youth lives with a caring adult who has been selected and approved by the transitional housing placement provider. A Single Site is where the youth lives on their own or with roommates in an apartment, condominium or single-family dwelling. Staff from the housing provider live on site.

What is Transitional Housing Placement-Plus Foster Care (THP+FC)?
The Transitional Housing Placement-Plus (THP+FC) is for Non-Minor Dependents (NMDs) who are 18 to 21 years of age and are in foster care. The purpose of THP-Plus is to provide transitional housing and supportive services based on a Transitional Independent Living Plan (TILP). Services and support include case management, supervision, and goal-oriented supportive services. Youth in THP+FC develop employment skills, financial management, and other daily living skills.

Who is THP+FC for?
THP+FC is for Non-Minor Dependents (NMDs), including those supervised by probation. An NMD is defined in Welfare and Institutions Code (WIC) section 11400 (v).

Where do the youth live?
Youth in THP+FC are either living with a Host Family, at a Single Site, or a Remote Site. A Host Family is where the youth lives with a caring adult who has been selected and approved by the transitional housing placement provider. A Single Site is where the youth lives on their own or with roommates in an apartment, condominium or single-family dwelling, rented or leased by the provider. Staff from the housing provider live on site. A Remote Site is where the youth lives in a single housing unit rented or leased by the provider. The youth lives independently but still receives regular supervision from the provider.

What is Transitional Housing Program-Plus (THP-Plus)?
The Transitional Housing Program-Plus (THP-Plus) is for young adults who are 18 to 23 years of age. Youth are eligible at or after their 18th birthday and are no longer eligible when they turn 24 or have been in the program for 24 cumulative months. Services and support include, stable housing, education assistance, counseling, employment support and training, and mental health services.

Who is THP-Plus for?
THP-Plus is for youth that are exiting or are no longer in foster care, including those supervised by probation.

Where do the youth live?
Youth in THP-Plus live in apartments, single-family dwellings, condominiums, college dormitories, or with Host Families. A Host Family is where the youth lives with a caring adult who has been selected and approved by the transitional housing placement provider.
